To license an animal, a valid rabies vaccination date must be on file
or a copy of the rabies certificate must be provided for each animal
in addition to filling out this form. Your license application will
not be processed without it. Include your name, address, telephone number,
and pet's name. Copies of rabies certificates can be sent via:

- You must apply for a special
animal permit if more than 2 dogs or 2 cats reside in your household.
- You must apply for a dangerous
animal permit for:
- Wolves
- Wolf-hybrid dogs (including Tundra Shepherds)
- Venomous snakes or snakes which present a risk of serious physical harm or death to humans
- Pit bulls (including Staffordshire Bull Terriers and American
Pit Bull Terriers) were banned from the City of Overland Park as of July 2006.

with the pet number and your contact information.
- Animal license fees are not refundable.
- Overpayment of animal license fees: No refunds will be processed unless the amount received is more than $6 over the amount due.
